We request that you disclose all medical conditions and/or prescription medications as there may be contraindications to some services.
An Infection Control Fee of $3.00 per day will be added to your final bill.
CANCELLATIONS, LATE ARRIVALS, OR MISSED (NO-SHOWS) APPOINTMENTS
We understand that things happen and unforeseen circumstances may arise at the last minute that may prevent you from making it to your appointment. However, a missed appointment not only delays your treatment, but also prevents us from offering that appointment to another client who could have been seen at that time.
We kindly request you notify our office at least 24 hours prior to your appointment of any cancellations or changes.
HALO Laser Resurfacing Appointments
Any changes or cancellations to a HALO appointment require a 48 hour advance notice. Failure to cancel or change less than 48 hours prior to the scheduled HALO appointment will result in the forfeiture of the appointment deposit in its entirety.
As a courtesy, our booking system will send you a confirmation email and/or text message 48 hours prior to your appointment date. Please ensure that we have your current mobile phone number and email on file so you can receive these confirmation/reminder messages.
PRODUCT RETURNS, EXCHANGES AND REFUNDS
We are committed to offering high-quality, scientifically proven products and want to ensure you are completely satisfied with your order. If you are not satisfied for any reason, we will accept returns within 14 days of your original purchase date. Shipping charges are not refundable.
◾ Gift Cards/Gift Certificates or Pre-paid Packages cannot be returned, exchanged or refunded.
◾ Fees paid for treatments/procedures are not refundable.
NSF/Returned Check Policy
A thirty-five dollar ($35.00) fee will be assessed for each check or bank draft that has been presented as payment for any service or product provided by Malisse Skin Bar and has been returned by the issuing bank unpaid.
Free standard shipping on all orders of $200 or more (excluding taxes and shipping charges) to all destinations in the continental U.S. except Alaska and Hawaii. A $15.95 shipping charge will be added to all orders less than $200 (excluding taxes and shipping charges) to all destinations in the continental U.S. except Alaska and Hawaii.
Skinade collagen boosting products are not eligible for free shipping.

If you are not 100% satisfied with your NUTRAFOL purchase, you can return the product and get a full refund or exchange up to 30 days from the date of purchase. Any product you return must be in the same condition you received it and in the original packaging.

APO/FPO/DPO Military Addresses
All shipments to APO, FPO and DPO addresses are sent via Smart Post. Due to military handling time, delivery to some destinations may take up to 21 business days.

How are your shipping charges calculated?
All orders receive free shipping. When available, you may expedite your order to ship next day for an additional charge.
Does SKINMEDICA deliver outside of the United States?
No. At this time, products can only be shipped within the United States.
How long will it take to deliver my order?
Orders are shipped Monday through Friday, excluding major holidays. Orders placed by 3:00 PM CT M-F will be shipped the same day. Orders placed after 3:00 PM CT M-F will be shipped the following business day.
What is SKINMEDICA's return policy?
SKINMEDICA is committed to offering high-quality, scientifically proven products and want to ensure you are completely satisfied with your order. If you are not satisfied for any reason, SKINMEDICA will accept returns within 30 days of your original purchase. All returns must be authorized through SKINMEDICA Customer Service and assigned a Return Merchandise Authorization Number (RMA).
Brilliant Distinctions® points and rewards that have been redeemed will not be reimbursed for returns.
How do I return an item I purchased online??
To initiate a return, please contact SKINMEDICA Customer Service. Once your request is processed, you will receive an email with a prepaid FedEx shipping label and a Return Merchandise Authorization form that must be included with your return. Shipping charges for returned products will be waived on all approved returns.
Use the prepaid FedEx shipping label and drop off your package at any FedEx location.
Can I return products I purchased online at the Spa?
In order to ensure prompt processing of your return, we highly encourage you to contact SKINMEDICA Customer Service to initiate a return.
When do I get my refund after I have returned an item?
If applicable, refunds will be provided once your product return has been received and processed. Refunds will be applied to the original payment card and may take up to 14 business days.
